School Based Family Advocates

​

Through collaboration and partnerships with some school districts within Franklin County, our Family Advocate Specialists can assist individuals and families with all social determinants of health. We will assist with financial budgeting, housing, food, assistance obtaining employment and education on resumes and interviewing, transportation, primary care connection, health literacy, health insurance enrollment, behavioral health connection, substance use connection, socialization and any environmental such as (court, probation, utilities ,personal hygiene products, clothing, furniture, household items, assistance understanding legal documents) Currently we have advocates in the Saranac Lake School District and the Chateaugay Central School District.
